Spinal arachnoid cyst compressing the spinal cord with syrinx

Diagnosis almost certain

Presentation

Progressive weakness of the lower limbs.

Patient Data

Age: 55 years
Gender: Male

Well-defined multiseptated posterior epidural cystic lesion of CSF signal on all sequences extending from T4 to T10 level with widening of the right neural foramina at T5-T6 level. A mass effect is noted on the adjacent spinal cord which is displaced anteriorly.

Central cystic cavity of the spinal cord of low signal on T1, and high signal on T2 and STIR, extending from C7 to T10 in keeping with a syrinx.

Segmental atrophy of the lower spinal cord below the syrinx.

Dural ectasia is noted.

Case Discussion

MRI features most consistent with a spinal arachnoid cyst of posterior epidural location compressing the spinal cord with proximal syrinx and distal spinal cord atrophy.
