Spinal ependymoma

Diagnosis almost certain

Presentation

Increasing difficulties with bilateral lower extremity pain. No associated bowel or bladder dysfunction.

Patient Data

Age: 30 years
Gender: Female

There is a T2 hyperintense, enhancing mass with several internal T2 hyperintensities within, likely representing cysts. The mass is of intradural extramedullary location at the level of L2 vertebral body, displacing the cauda equina anteriorly, without obvious signal change or enhancement of the nerve roots.

Case Discussion

MRI features most consistent with spinal ependymoma. The patient had lumbar laminectomy at L1-2 and L2-L3 with complete resection of the mass. The histopathology revealed a myxopapillary ependymoma, WHO grade II.

There has been no sign of recurrence on surveillance MRIs. The patient is doing well without recurrence of her original symptoms.
