Spinal meningioma

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Chronic dorsal pain.

Patient Data

Age: 75 years
Gender: Male
ct

A small partially calcified mass (25 x 14 x 10 mm) of extramedullary location is seen in the left lateral aspect of the spinal canal at the level of T8-T9 level, displacing the spinal cord to the right.

mri

The MRI sequences show a well-circumscribed intradural extramedullary mass at T8-T9 level of left lateral location. It elicits an intermediate signal on T1, high signal on T2 with an area of low signal (calcified component) with heterogeneous enhancement on postcontrast sequences. The adjacent segment of the spinal cord is compressed and displaced to the right with no abnormal intrinsic signal.

Case Discussion

CT and MRI features are most consistent with a spinal meningioma (partially calcified)

The thoracic region is considered as the most frequent location of the spinal meningiomas. Calcification may be present but uncommon.

On imaging, the differential diagnosis should include neurogenic tumors (spinal schwannoma and spinal neurofibroma).
