Spinal schwannoma - dumbbell appearance

Presentation

Paraparesis of the lower limbs.

Patient Data

Age: 30 years
Gender: Female

There is a well-defined intradural extramedullary soft tissue mass of the spinal canal, extending through the left T8-T9 neural exit foramen, forming a dumbbell-shaped mass. It is of intermediate signal intensity on T1WI and T2WI with areas of high signal on T2WI and heterogeneous enhancement on postcontrast T1WI fat sat sequences. Widening and remodeling of the left neural foramen are noted. The adjacent segment of the spinal cord is compressed and displaced anterolaterally.

Case Discussion

MRI features suggestive of a neurogenic tumor, most likely a schwannoma, although the differential includes neurofibroma, ganglioneuroma or less likely meningioma.
