Right hip pain and immobility

Age: 40 years
Gender: Female
mri

There is a 5.5 x 11 x 21 cm extensive large infiltrative soft tissue mass involving the gluteus maximus, medius, minimus and adductor group muscles.

The right sciatic nerve also show abnormal intrasubstance increased signal and caliber due to nerve involvement.

Case Discussion

This is a pathologically-proven case of low-grade spindle cell sarcoma.

Microscopic:

Sections show neoplastic tissue composed of a diffuse proliferation of spindle cells with eosinophilic cytoplasm, prominent nucleoli, minimal atypia and rare mitotic figures (1-2/10 HPF) growing in long sweeping fascicles and infiltrating the skeletal muscle.
