Spinous process metastasis in prostate cancer

Case contributed by Roberto Schubert
Diagnosis almost certain

Presentation

Back pain. Radical prostatectomy 4 years earlier. Elevated PSA.

Patient Data

Age: 80 years
Gender: Male
Nuclear medicine

High osteoblastic activity in the dorsal parts of the 8th thoracic vertebra.

Dorsal spine

ct

CT shows osteosclerosis of the spinous process of T8. No osteolysis or destruction.

The bright dot in the T10 vertebral body is probably an enostosis, for it shows no activity in the bone scan.

Dorsal spine

mri

Low signal of the spinous process of T8 on T1-weighted images, only minimally increased signal on STIR compared with the other vertebrae.

Case Discussion

Imaging features are consistent with spinal metastasis affecting the spinous process of T8 and incidental enostosis of T10 vertebral body.
