Presentation

The patient presents for an operative workup for severe halluces valgi.

Patient Data

Age: 70 years
Gender: Female
x-ray

There are severe halluces valgi, with bilateral metatarsus primus varii. There are laterally subluxed bases of the proximal phalanges of the first digits in relation to the first metatarsals and consequent increased first metatarsal-phalangeal angles. There is developing secondary osteoarthritis of the first MTP articulations with joint space narrowing, remodeling and expansion however no medial spurring is apparent. There are laterally subluxed hallux sesamoids. There are bilateral bunions.

There is evidence of a splayfoot bilaterally with left 2nd MTP subluxation, right 2nd and 3rd digit hammer toes, and suspected left 3rd digit hammer toes. There are in addition bilateral tailor's bunions (metatarsus quintus valgus), with a widened 4th-5th intermetatarsal angle, regional soft tissue and 5th metatarsal head prominence.

There is age-appropriate osteoarthritis and degenerative calcaneal spurring.

x-ray

The contrast adjusted and zoomed views confirm the right 2nd and 3rd digit hammer toes and suspected 3rd digit hammer toe.
