Presentation

Left hypochondrial pain.

Patient Data

Age: 60 years
Gender: Female

Irregular shaped lesion implicating the spleen showing air/fluid levels, multiple air locules within, and faint peripheral enhancement.

Hepatomegaly and mild perihepatic fluid collection.

Left-sided pleural effusion.

Case Discussion

Splenic abscesses are localized collections of inflammatory necrotic collection caused by many pathogens (bacteria, fungi, or parasites). Patients usually present with constitutional symptoms (fever, rigors, chills) and left hypochondrial tenderness.

CT usually shows hypodense fluid collection with faint peripheral enhancement, the presence of gas density within confirms the diagnosis.
