Splenic autolysis with pseudocyst formation

Presentation

Chronic left hypochondriac region pain,low grade fever,weight loss,generalized weakness.

Patient Data

Age: 58
Gender: Male
Modality: Ultrasound

Welldefined,smooth,thin walled,iso-hypoechoic cystic structure noted in the left hypochondriac region with mobile internal echoes.The splenic vessels can be seen converging towards the medial aspect of this cystic structure.Some solid intramural component noted along the medial aspect of the structure.Spleen not visualized separately from this cystic structure.Rest of the abdominal and pelvic organs appear normal. 

Modality: CT

Large,well defined,smooth walled,iso-hypodense cystic structure noted in the left hypochondriac region.No internal or wall calcification.No internal fat density.No internal hemorrhage.Spleen not visualized separately from this cystic structure.

Modality: CT

Well defined,smooth,thin walled cystic structure noted in the left hypochondriac region.Homogeneous internal density of 13-17H.U noted.Small,moderately enhancing solid intramural component noted along the medial aspect.Mild pericystic fat stranding noted.Left sided mild pleural effusion seen.Spleen not visualized separately at its normal anatomical position nor at other ectopic sites.Rest of the abdominal and pelvic organs appear normal.

Case Discussion

Blood examination :

Hypochromic microcytic anemia,increased leucocyte count,markedly increased platelet count,abnormal neutrophils,immature myeloid precursors s/o Dysmyelopoiesis.

Bone marrow examination:

Dysmegakaryopoiesis with hyperplasia s/o Myeloproliferative disorder ?Chronic myeloid leukemia.

Histopathological report: Splenectomy was done,

Enlarged spleen with autolysis and pseudocyst formation.

Types of Splenic cyst:

Congenital:haemangioma,lymhangioma,epidermoid cyst.

Infective:Hydatid cyst,pyogenic abscess.

Traumatic pseudocyst.

Intrasplenic pancreatic pseudocyst.

Cystic metastasis.
