There are varicosities along the gastric fundus and splenic hilum that correspond to an abnormal collateral portosystemic communication between the splenic vein and left renal vein. Splenomegaly is also present (430 cm³). Splenic vein is preserved. 

Case Discussion

This case shows a portocaval collateral system developed as a reaction of the vascular system to high portal pressure. This system induces a decrease in portal pressure with blood bypassing liver direct to IVC which may result in complications such as hepatic encephalopathy.
