Presentation

Presents with lower limb weakness and numbness with a valgus foot deformity. Hypertrichosis was observed at the lower lumbar region.

An osseocartilagenous median septum is seen completely traversing the spinal cord at the lower lumbar level splitting the cord into two unequal hemicords. The spinal cord is seen extending till the sacral segments showing evidence of hydrosyrinx.

Split cord malformations (SCM) is a new term used for diastematomyelia and diplomyelia. It is a form of occult spinal dysraphism, which is defined as skin covered lesion that have no exposed neural tissue and no visible cystic mass, barring few cutaneous anomalies, 

SCM are of two types:

  • type 1 consists of two hemicords, each contained within its own dural sheathed tube and separated by a median bony spur (this case) 
  • type 2 consists of two hemicord housed in a single dural tube separated by a fibrous median septum
