L4-5 inflammatory process involving the vertebral end plates with associated destruction of the VEPs, marginal osteophytes with enhancement in post contrast images and extension of the enhancement to the paraspinal region. The disc substance is degenerated and shows diffuse annular bulge, obliterating the epidural fat and indents the ventral aspect of the thecal sac.
