Spondylodiscitis - post surgery

Case contributed by Dr Bruno Di Muzio

Presentation

Inpatient one month after a lumbar spine surgical procedure that evolved with right L4 radicular pain and fever.

Patient Data

Age: 50-year-old
Gender: Female

Lumbar spine

Modality: MRI

L4/5 spondylodiscitis with intervertebral disc height loss, endplates irregularities and enhancement, as well as soft tissue oedema and enhancement involving both psoas muscles and the epidural space on the right at this level.

Case Discussion

This patient was diagnosed with a post-surgery spondylodiscitis associated with local spinal/paraspinal abscess on the right of L4 level (previous scans not shown). The collection was drained and the patient has presented clinical improvement since then. The current MRI scan still demonstrates a diffuse paravertebral and psoas muscles oedema, as well as the signs of spondylodiscitis.
