Spondylolysis of L4 and L5

Presentation

Low back pain.

Patient Data

Age: 30 years
Gender: Male
ct

Bilateral L4 and L5 pars interarticularis defects are noted in keeping with spondylolysis. No associated anterolisthesis is seen.

L4-L5 disc bulge is noted.

Incomplete closure of the posterior neural arch of S1 (spina bifida occulta).

CT features of bilateral L4 and L5 spondylolysis with associated spina bifida occulta of S1.

Spondylolysis may be the result of a repeated microtrauma, causing a stress fracture of the pars interarticularis. Usually associated with dysplastic pars. Genetics may be a predisposing factor.
