An AP chest radiograph was inadvertently saved back to front, and then the orientation marker placed on the wrong side, and only by reviewing previous images was it clear that this patient actually had levocardia.
The radiographer was alerted and the image was flipped so that the orientation was correct.
Case Discussion
This case illustrates the importance of checking previous images as the initial image viewed in isolation gives the impression that the patient's heart lies on the right.
