Case contributed by Kamel Mahmoud Alkhateeb
Diagnosis almost certain

Presentation

Vaginal bleeding and dyspareunia

Patient Data

Age: 40 years
Gender: Female

There is mass lesion arising from the posterior wall of the cervix, exhibiting intermediate signal on T1WIs and T2WIs with diffusion restriction. It shows enhancement on post contrast images. The mass lesion extends to posterior fornix of upper third of vagina. The mass lesion measures about 4 x 3 x 2.5 cm in maximum dimensions. Loss of normal T2 hypointense signal of left posterior cervical margin is seen with adjacent stranded fat plane, suggesting focal parametrial extension, however no evidence of invasion to rectovaginal septum. The fat plane between the cervix and bladder anteriorly looks normal. No pelvic lymphadenopathy.

Small posterior subserosal uterine calcified fibroid is seen.

Case Discussion

Colposcopy guided biopsy of posterior lip of cervix:

Pathology result: Squamous cell carcinoma, HPV associated, Grade -2.
