Stener lesion

Diagnosis almost certain

Presentation

Fell on the ground resulting in left thumb injury a few days back. Complaining of the thumb base pain, swelling on ulnar side.

Patient Data

Age: 50 years
Gender: Male
ultrasound

A normal ulnar collateral ligament is absent at the thumb MCP joint. There is a hypoechoic pseudomass at the distal end of the metacarpus with a bone fragment in it. The proximal phalanx base shows a corresponding defect. The radial collateral ligament of the thumb is normal.

The contralateral thumb ulnar collateral ligament was examined for comparison which was normal.

1st cine-loop shows symptomatic thumb MCP region during interphalangeal joint flexion and extension. 2nd cine-loop shows the contralateral thumb.

Case Discussion

An adult male had a thumb injury due to a fall on the ground. The initial x-ray ( no copy-right, not uploaded) showed a displaced fracture at the thumb MCP joint on the ulnar side raising a possibility of a Stener lesion. The ultrasound shows a Stener lesion which is avulsed and displaced ulnar collateral ligament superficial to the adductor aponeurosis.
