Presentation

Long standing constipation and acute abdominal pain

Patient Data

Age: 90 years
Gender: Male

Colon is dilated and full of fecal material. The wall of the colon is thin, except for the rectosigmoid wall that is thickened. On the non-contrast CT, rectosigmoid mucosa is hyperdense. There is also hyperenhancement in the same location. Peri-colic fat stranding and stercoral ulcers are also noted. The presence of peritoneal fluid suggests stercoral peritonitis. The presence of pneumoatosis and discontinuation in the enhancement of rectal wall is suggestive of ischemic complication. 

Case Discussion

The patient underwent proctosigmoidectomy. The surgeon reported the presence of many peritoneal adhesions, a finding that we attributed to recurrent microperforation.
