Strangulated umbilical hernia

Diagnosis almost certain

Presentation

Signs of small bowel obstruction with a palpable hard lump at the umbilical region. History of cholecystectomy 25 years ago.

Patient Data

Age: 80 years
Gender: Female

Umbilical hernia containing a small bowel loop which shows a C-shaped configuration, thick wall and feces sign with surrounding mesenteric fat stranding and dilated proximal small bowel loops.

Another defect of the midline anterior abdominal wall above the umbilicus containing a part of the transverse colon is noted.

Bilateral inguinal hernia of epiploic content.

Bilateral parapelvic cysts are noted

Case Discussion

The clinical presentation and the CT features are highly suggestive of a strangulated umbilical hernia, which was confirmed at surgery and the patient had a segmental small bowel resection with umbilical hernia repair.
