Stress fracture of the second metatarsal bone

Case contributed by Dr Balint Botz

Pain in the left forefoot worsened by weight bearing, that began immediately after a day of intensive basketball playing. No specific injury mechanism reported.

Patient Data

Age: 20 years
Gender: Female

X-rays obtained upon the first presentation

No fracture line was discernible in the initial x-rays. Nevertheless, considering the high clinical probability of metatarsal stress fracture a therapy with lightweight cast was initiated.

Control x-rays one month later

A healing stress fracture is visible through the lateral cortex of the second metatarsal with marked periosteal bone formation.

Case Discussion

Typical presentation of metatarsal stress fracture, where the pathology was occult in the initial x-rays.
