Severe shoulder pain and swelling after lifting a heavy object.

Significant fluid collection in the subacromial-subdeltoid bursa, as well as an adjacent ill-defined fluid tracking below the humeral metaphysis and around the subscapularis muscle, highly suggestive of subacromial-subdeltoid bursa rupture.
Incidental findings of supraspinatus tendinitis and acromioclavicular joint osteoarthritis are noted.
Case Discussion
Subacromial-subdeltoid bursa rupture is a rare traumatic injury. The most striking finding is the ill-defined shape of the bursa and extension of the fluid beyond the proximal humeral metaphysis and around the subscapularis muscle.
