Presentation
History of left brain trauma 20 days ago. The patient has left side weakness and deteriorate mental status.
Patient Data
Isodense, subdural collection along the right fronto-parieto-temporal lobes with resulting mass effect over the right cerebral hemisphere, effacement of adjacent sulci and right lateral ventricle as well as mild midline shift towards the left side.
Case Discussion
Subdural hematomas are mainly due to rupture of bridging cortical veins following trauma. They have a crescent shape and cross the cranial sutures. Large hematomas can cause mass effect as in this case.
The age of the blood on CT can be estimated as acute (hyperdense), subacute (isodense) and chronic (hypodense) relative to adjacent gray matter.
