Subarachnoid hemorrhage and left anterior cerebral artery aneurysm

Presentation

Patient came to the ER department with history of sudden onset of severe headache for 1 hour, frequent projectile vomiting and fainting attack with loss of consciousness for 30 minutes . Patient had headache since 5 days but deteriorated today he was drowsy with mild neck stiffness. CNS examination : pupil RRR, left sided weakness, motor power 4/5, positive Babinski sign on left side.

Gender: Male

CT brain with IV contrast showing saccular aneurysm at ACA. There is also subarachnoid space hemorrhage in non contrast images.
