Presentation
Initial screening mammogram followed by a diagnostic study
Patient Data
Two CC images of the left breast 2 years apart.
CC view left breast shows very subtle spiculated density in the subareolar breast tissue. It was read as normal.
CC view of the same breast two years later. Second image annotated.
Obvious spiculated mass in the subareolar left breast.
Case Discussion
This case demonstrates the development of a carcinoma in the subareolar breast tissue.
The area is one of the review areas in the breast; an area that you consciously have to review before you move on. Another such area is the submammary fat on the MLO view. Cf. case 21726
On ultrasound of these lesions, do not be caught out by the acoustic shadow that is normally present behind the nipple. Angle your transducer to peer in behind the nipple. This case illustrates why the ultrasound tech needs to look at the mammogram images before the ultrasound is done. The ultrasound tech needs to know what he/she is looking for before the ultrasound is done. Breast imaging is a team effort.
