Subchondral femoral condyle fractures - hyperextension injury

Presentation

Right knee injury.

Patient Data

Age: 16 years
Gender: Male

Lateral femoral condyle depression. No discrete fracture. Small joint effusion.

Medial and lateral femoral condyle marrow oedema with low T1 signal. Mildly depressed lateral sulcus terminalis subchondral fracture. Medial femoral condyle subchondral fracture with a wavy appearance of the medial trochlear cortex and chondral high signal reflecting injury.

Central lateral tibial plateau marrow oedema with bone contusion and incomplete fracture.

No meniscal or ligamentous injury identified.

Case Discussion

In keeping with the hyperextension mechanism of injury, there are bone bruises with subchondral fractures of both femoral condyles and bone bruise/incomplete fracture of the lateral tibial plateau.
