Diagnosis almost certain

Presentation

Dizziness after head trauma a month prior. Bleeding? Infarction?

Patient Data

Age: 60 years
Gender: Male

A cortex-isodense lesion is seen in the right frontal lobe indenting the frontal horn of the right lateral ventricle. No evidence of surrounding edema, intracranial bleeding or infarction.

Nodular heterotopic grey matter is seen within the white matter of the right frontal lobe extending from the lateral ventricle to the overlying cortex and indenting the frontal horn of the right lateral ventricle. There is slight volume loss of the affected lobe when compared to the other side. Corpus callosum appears unremarkable. 

Case Discussion

Mixed nodular and curvilinear subcortical heterotopia of the right frontal lobe.

No history of epilepsy.

No evidence of epileptogenic foci in EEG.
