Subcutaneous dirofilariasis

Presentation

2-month history of a painless mobile tiny lump on the medial proximal left lower leg; 2 months before this a pinprick focus of erythema was noted at the same site, but ignored by the patient.

Patient Data

Age: 30 years
Gender: Male

A small cyst is seen in subcutaneous tissues of proximal left lower leg medially. An internal tubular serpiginous structure is seen, with parallel echogenic walls and an anechoic center.

The serpentine structure shows active movements/ undulations, referred to as 'filarial dance sign'.

Case Discussion

Dirofilariasis is primarily a zoonosis caused by Dirofilaria spp., mainly Dirofilaria repens. The primary reservoirs of D. repens are canines, with humans as accidental hosts. Most of human dirofilariasis is pulmonary or ocular infections, although subcutaneous nodules may also be seen. Patients with subcutaneous infection notice a lump at the affected site. The common sites are the face, trunk, arms, thighs, legs and male genitalia.
