Subcutaneous neck abscess

Presentation

Focal pain and swelling in the left neck.

Patient Data

Age: 40 years
Gender: Female
ct

Small subcutaneous left IIb triangle abscess measuring 7 x 12 x 23 mm (LL x AP x CC). Extensive perilesional cutaneous thickening and subcutaneous fat infiltration abutting the parotid gland - phlegmon.

Left jugular chain lymph nodes not enlarged but hyperenhancing (cf. right chain).

Case Discussion

One day history of left postauricular inflammatory, painful mass with a central punctum.
Normal ear and oral exam, no trismus.

Impression:
Postauricular phlegmon/abscess may represent an insect bite/streptococcus or staphylococcus infection.

The patient was started on intravenous amoxicillin/clavulanic acid.
The CT scan confirmed a large phlegmon with a small central abscess.
The abscess was incised and drained. Pus culture yielded S. aureus.
