Subdural empyema

Presentation

Severe headache and fever with drowsiness for a few days.

Patient Data

Age: 14 years
Gender: Male

Large subdural collection along the left frontoparietal region returning low signal on T1WI and high signal intensity on T2WI. The collection enhances peripherally with adjacent dural and leptomeningeal enhancement as well as a restriction pattern on DWI and corresponding ADC map. The collection is causing mass effect over the left cerebral hemisphere.
 

Case Discussion

Subdural empyema is an extra-axial intracranial collection that accounts for 20-33% of the intracranial infectious processes 1.

The most common cause is sinusitis, which is causing a severe condition which needs emergency intervention. Empyema is less common in the post-traumatic patient due to secondary infection as well as a postsurgical infection which has a mild clinical course 1.
