A large acute on chronic subdural hemorrhage is seen on the left exerting marked mass effect on the underlying left hemisphere with midline shift, subfalcine and uncal herniation. The right lateral ventricle is dilated. Blood is also seen layering on the tentorium.

Typical appearances of a large subdural hematoma resulting in marked mass effect. The patient went on to have a craniotomy and evacuation and made an uneventful recovery.
