Presentation

Increasing headache and altered mental status after recent fall. History of incidental chronic subdural hematoma in the past. Evaluate for hemorrhage.

Patient Data

Age: 70 years
Gender: Male

There is a mixed density holohemispheric subdural hematoma on the right. There is resultant effacement of the subjacent sulci and right lateral ventricle. There is a leftward midline shift.

Status post drain placement

There are postoperative changes from the interval placement of a subdural drain in the right frontoparietal convexity, with a subsequent decrease in size of the right holohemispheric subdural hematoma and leftward midline shift.

Case Discussion

This is a case of a large (acute on chronic) subdural hematoma with significant improvement following the placement of a subdural drain. The patient tolerated the procedure well and was eventually discharged once the subdural drain was removed.
