Subependymal heterotopia - cyst of the velum interpositum - syrinx

Diagnosis almost certain

Presentation

Had surgery for cervical myelomeningocele three years ago.

Patient Data

Age: 4 years
Gender: Male

There is a posterior midline cystic structure measuring 4.3 cm in transverse diameter, bulging into the posterior body of the lateral ventricles. It is located above and anterior to the pineal gland, and inferior to the fornices which are displaced superiorly in keeping with a cyst of the velum interpositum.

There are small subependymal nodules distorting the outline of the ventricles, isointense to grey matter on all sequences in keeping with subependymal grey matter heterotopia.

A cystic lesion is noted within the cervical spinal cord extending from the bulbo-medullary junction up to C6 level measuring 1.6 cm in diameter and 5 cm in length, in keeping with syrinx.

The repaired myelomeningocele was at the C3-C4 level.

Case Discussion

MRI features of bilateral subependymal grey matter heterotopia with cyst of the velum interpositum and syrinx of the cervical spinal cord.
