Subperiosteal abscess of the orbit

Diagnosis almost certain

Presentation

Left orbital pain, progressive left side eye proptosis and limitation of movement, upper eyelid soft tissue swelling, and low-grade fever for five days.

Patient Data

Age: 15 years
Gender: Male

Fluid and thick mucosa in all paranasal sinuses with sinus cavities obliteration more pronounced in the left side and left orbital nasal side subperiosteal abscess containing small gas bubbles with axial width up to 33 x 15 mm and height up to 25 mm with impression on related medial rectus muscle and superior oblique muscle and left eye proptosis, exodeviation, and preseptal and lacrimal gland soft tissue swelling are seen. Right eye exodeviation and mild retrobulbar inflammatory change on the left side are also noted.

Case Discussion

The case illustrates non-contrast MDCT features of the pus aspiration-proved subperiosteal abscess of the orbit due to infective sinusitis.
