Subpulmonic effusion

Diagnosis almost certain

Presentation

Pleuritic right chest pain for one day. Cough, weight loss and night sweats for past month. Positive family history for pulmonary tuberculosis.

Patient Data

Age: 20 years
Gender: Female

Chest X-ray

x-ray

There is blunting of right costophrenic angle with elevation of right pseudo-hemidiaphragm suggestive of right subpulmonic effusion.

Reticulo-nodular changes are seen in bilateral upper lobes with some fibrotic changes note on left. There is a cavitating nodule seen at right base. 

Case Discussion

The presence of a subpulmonic effusion was confirmed with a limited ultrasound of the right hemithorax.

In consideration with positive family history and CXR findings, there is high suspicion of pulmonary tuberculosis which was later confirmed by sputum gene testing.
