Presentation

Primary infertility.

Patient Data

Age: 35 years
Gender: Female

Pelvis

Partial septate / subseptate uterus.

Incidental small posterior subserous fibroid.

Transvaginal 3D

Intact fundal contour of the uterus. The fundal myometrial depth measures 1.2 cm, more than that of the arcuate uterus and less than that of the septate uterus. The angle between the two uterine horns is a little less than 90° suggestive of partial septate / subseptate uterus.

Case Discussion

Ultrasound and MRI features are consistent with a partial septate / subseptate uterus and incidental subserosal fibroid. A partial septate uterus involves the endometrial canal but not the cervix, with the angle between the endometrial horns less than 90° and the fundal myometrial depth >1 cm and <1.5 cm. Differential diagnosis is arcuate uterus and bicornuate uterus.
