Subseptate uterus

Presentation

Unmarried female with pelvic pain for 1 year.

Patient Data

Age: 30 years
Gender: Female
mri

MR imaging shows two endometrial cavities separated by a fibromuscular septum with the acute angle between the two horns. The septum is not extending into the internal OS.

A T1  heterogeneously hyperintense lesion with T2 shading is noted in the right ovary consistent with endometrioma. Another tubular T1 hypo/T2 hyperintense lesion noted in the right adnexa suggests hydrosalpinx.

T1 hyperintense tubular structure with T2 shading in the lower aspect noted in left adnexa suggest tubal endometrioma. The patient had a prior history of left-sided oophorectomy.

Case Discussion

Two endometrial cavities with mixed septum not extending to the internal OS, acute angle noted between the two horn without fundal indentation consistent with subseptate uterus. It is a case partial septate uterus since the septum is not extending into internal OS. Close differential diagnoses are bicornuate and arcuate uterus.

A bicornuate uterus is a fusion anomaly where two uterine horns are widely separated with an obtuse angle and fundal indentation.

The arcuate uterus is a milder form of resorption anomaly where broad indention is noted within the endometrial cavity without a fundal cleft.
