Superficial cerebral vein thrombosis

Case contributed by Omar Giyab
Diagnosis almost certain

Presentation

Patient presented with sudden onset of severe headache

Patient Data

Age: 31
Gender: Female

A crescentic shaped intraparenchymal bleeding was seen at an atypical location in the right parietal lobe. A hyperdense superficial cerebral vein is seen adjacent to the bleed, along the convexity leading to the superior sagittal sinus.

CT venography was performed to rule out superior sagittal vein thrombosis. There was no thrombus seen in the superior sagittal vein.

Case Discussion

Intracerebral bleeding with an atypical location should raise the suspicion of venous thrombosis, vascular malformation or tumor. It is ideal to perform a post contrast phase study or venography to investigate for the underlying etiology.
