Superior mesenteric artery syndrome

Presentation

Pain and fullness in abdomen.

Patient Data

Age: 15
Gender: Female

Compression of the third portion of the duodenum at the midline between the aorta and superior mesenteric artery, resulting in proximal dilatation of the duodenum and stomach.

Aorto-mesentric angle = 15 degrees

Aorto-mesentric distance = 4.5 mm

Case Discussion

Narrowing between the superior mesenteric artery and aorta can result in compression of the third portion of the duodenum, leading to obstruction. This is referred to as superior mesenteric artery (SMA) syndrome / Wilkie syndrome.

Specific parameters which help to diagnose SMA syndrome, found in this case:

  • compression of the third portion of the duodenum
  • reduced aorto-mesentric angle between 6° to 15°
  • reduced aorto-mesentric distance between 2 to 8 mm.
