Superior peroneal retinaculum avulsion fracture

Presentation

Peristent lateral ankle pain after a sprain injury. Reportedly no fracture signs on radiographs.

Patient Data

Age: 45 years
Gender: Male

Bone edema at the lateral dorsal aspect of the lateral malleolus. Hemorrhage around the peroneal tendons in the retromalleolar groove. The longus tendon shows lateral subluxation.

Small cortical avulsion at the dorsal lateral malleolus, corresponding to the bony insertion of the SPR.

Case Discussion

This type of injury may lead to chronic peroneal tendon instability and subluxation.
