Large suprasellar cystic lesion of CSF intensity can be seen. Dilated lateral ventricles suggest an associated obstructive hydrocephalus. The cyst markedly stretches the optic nerves, chiasm and tracts. 

Case Discussion

This case demonstrates the typical appearance of a large suprasellar arachnoid cyst. It is important to distinguish from obstructive hydrocephalus with marked dilatation of third ventricle, as treatment options differ and misdiagnosis could lead to patient deterioration. Shunting of the lateral ventricles in this case can actually precipitate enlargement of the cyst.
