Suprasellar arachnoid cyst and tuberous sclerosis

Age: Child

Suprasellar arachnoid cyst is seen indenting the floor of the 3rd ventricle with subsequent mild supratentorial hydrocephalus.

Multiple subcortical high T2 and FLAIR lesions, suggestive of cortical tubers.

Multiple small subependymal nodules along the body of both lateral ventricles.

Features are consistent with tuberous sclerosis.

Case Discussion

This child not only has tuberous sclerosis but also a suprasellar arachnoid cyst. This should not be confused with obstructive hydrocephalus with dilatation of the third ventricle, as shunting can result in blindness as the cyst suddenly expands.
