Supraspinatus tendon tear - arthrographic diagnosis

Case contributed by Dr Chris O'Donnell

Presentation

Chronic shoulder pain ? capsulitis. For hydrodilatation

Patient Data

Age: 60 years
Gender: Female

Contrast leak through a full-thickness supraspinatus tendon tear - this is the "old fashion" way of diagnosing this pathology.  One can observe the markedly distended subacromial/subdeltoid bursa whilst the arrow points to a defect in the supraspinatus tendon with contrast leak into the subacromial bursa. Hydrodilatation will not be successful at alleviating symptoms as the injected fluid decompresses immediately from the joint into the bursa leading to bursal distension.
