Presentation

Carcinoma right breast 6 years ago. Follow up mammography and ultrasound.

Patient Data

Age: 70 years
Gender: Female

The mammogram shows the post therapy appearance of the right breast following surgery and radiation.

You noticed the small area of fat necrosis centrally (arrowed) in the scar on the MLO view, right?

Ultrasound images of the area near to the surgical scar, show a hypoechoic region with lobulated margins and posterior shadowing.

Case Discussion

The ultrasound images can be quite intimidating at first glance but don't be phased by the anechoic appearance of the surgical scar. They often look like this. The secret in following up these patients is to conscientiously compare imaging on follow up with previous studies. Except for fat necrosis and skin changes related to radiation, once imaging is stable be very leery of any changes in follow up.
