Type A dissection surgically corrected by replacement of the ascending aorta. Creation of a graft from the ascending aorta to the brachiocephalic and left common carotid artery origins (left common carotid origin tied off) due to involvement by dissection. Note more distally the intima is perforated with contrast entering the false lumen. The right kidney is small and poorly perfused due to its origin for the false lumen.
Note on the axial series (arterial) the origin of the brachiocephalic graft (arrow) from the replaced ascending aorta. The subtle defect in the intima of the descending thoracic aorta into the false lumen, the celiac axis arising from the true lumen and the left renal artery from the true lumen and right from the false
