Suture abscess post appendicectomy

Presentation

Post appendicectomy day 7. Patient complained of localised pain and tenderness in right lower quadrant. No fever.

Patient Data

Age: 16 years
Gender: Female
Modality: Ultrasound

Post laproscopic appendicectomy. Post op day 7.

Paired linear echogenicity is present in right lower quadrant arising from caecum.  No vascularity or aliasing present over it.  Minimal fluid is present around it. Mild mesenteric fat inflammation is also present in this area. No loculated collection or abscess visualized.

Modality: CT

CT was performed which did not reveal any radio opaque foreign body. Fat stranding with minimal fluid in RLQ.

Case Discussion

Case was discussed with surgeon.

The paired linear opacity could represent long tag of suture material. Residual suture after an operation can give rise to local irritation causing inflammation of surrounding fat with fluid. This can further progress to abscess formation if it gets infected. The patient will complain of localized pain, tenderness and fever.
