Suture abscess post appendectomy

Diagnosis probable

Presentation

Post appendectomy day 7. Patient complained of localized pain and tenderness in right lower quadrant. No fever.

Age: 16 years
Gender: Female

ultrasound

Post laproscopic appendectomy. Post op day 7.

Paired linear echogenicity is present in right lower quadrant arising from cecum.  No vascularity or aliasing present over it.  Minimal fluid is present around it. Mild mesenteric fat inflammation is also present in this area. No loculated collection or abscess visualized.

CT was performed which did not reveal any radio opaque foreign body. Fat stranding with minimal fluid in RLQ.

Case Discussion

The case was discussed with surgeon.

The paired linear opacity could represent long tag of suture material. Residual suture after an operation can give rise to local irritation causing inflammation of surrounding fat with fluid. This can further progress to abscess formation if it gets infected. The patient will complain of localized pain, tenderness and fever.
