Synovial facetal cyst

Presentation

Left sided back pain

Patient Data

Age: 70 years
Gender: Female

Grade I anterolisthesis of L4 over L5 vertebra body is seen with possible bilateral spondylolysis. Concentric bulge, annular tear of L4-5 disc is seen with ligamentum flavum thickening and facetal arthropathy. Cystic lesion measuring approx. 7 x 10 mm is seen at anteromedial aspect of left L4-5 facet joint, protruding into the spinal canal. This along with L4-5 disc is causing severe compression of the central canal, left traversing L5 nerve root , left lateral recess and mild compression of bilateral neural foramina.

Case Discussion

Synovial facetal cyst at L4-5 level with other degenerative spondylotic changes responsible for patient's complaints. Fatsat MR sequences are not available, but would have been useful in delineating the synovial cyst better.
