Synovitis - knee

Diagnosis almost certain

Presentation

Knee pain and swelling.

Patient Data

Age: 10 years
Gender: Male
mri

The synovium is abnormally thickened returning low signal on both T1 and T2 weighted image with intense post contrast enhancement. Mild reactive joint effusion is noted.

There is stranding of the Hoffa's fat pad.  Subtle thinning of the tibiofemoral and patellofemoral articular cartilage is noted.

Multiple enlarged lymph nodes are seen along the popliteal lymphatic chains.

Case Discussion

Radiographs are insensitive for detection of the synovial changes. Ultrasonography and MRI proper imaging modalities for evaluation of the synovium. Synovitis can be secondary to inflammatory of infectious processes.
