Diagnosis almost certain

Presentation

H/O penetrating injury to dorsum of 3rd MCP joint by ? thorn. F/B local pain and swelling.

Patient Data

Age: 40 years
Gender: Female

No bony abnormality.

ultrasound

Synovial thickening is noted involving 3rd metacarpo-phalangeal joint along with mild effusion.

Hypervascularity is noted in the thickened synovium - Synovitis of 3rd metacarpo-phalangeal joint.

An echogenic focus is noted embedded in synovium on dorsal - ulnar aspect; at neck of 3rd metacarpus. Size is 1 mm only - ? Foreign body embedded in synovium.

No flexure / extensor tendon tenosynovitis or tear is noted.

No bone erosion is noted.

An ossicle is noted near the joint.
