Presentation

Patient with non-specific pelvic pain underwent pelvic US which revealed a cyst with a subsequent referral for an MRI.

Patient Data

Age: 35 years
Gender: Female

A large presacral cystic lesion is seen projecting to the right side, displacing the rectum to the left side, and abutting the posterior aspect of a retroverted uterus. Small daughter cysts are noted along its posteromedial surface. The large cyst elicits intermediate signal on T1, slightly bright signal on T2 with diffusion restriction on DWI. The daughter cysts show a bright T1 signal. There is a marginal enhancement of the cyst wall following IV contrast administration.

Case Discussion

The patient underwent surgical excision which proved the tailgut duplication cyst lined by squamous epithelium and filled with mucinous materials. No hemorrhage or pus inside. No malignant transformation.
